The anime will try to make it seem ambiguous whether it's real or a hallucination, then it'll all come down to whether Makoto believes it's real (and kills himself) or believes it's a hallucination (and kills kyoko or P4 protag or something). Penultimate episode will be ambiguous. He'll make a choice and it'll cut to black.

I'm betting that(real) Chiaki set this up as a VR simulation, and is using a copy of Junko to run the game. Chiaki wants to expose Munakata and his Mirai friends as being less the cure to the world's despair, but instead the root of it.
Old man and the teacher probably helped to create and evolve Junko into what she became. Rest of the cast probably has their own crimes as well.
Chiaki gets to expose the Future Foundation as being just as evil as the Despair they claim to fight, Junko gets one last opportunity to fuck with Naegi. The world gets to see their 'saviors' being a bunch of kill happy psychos while Naegi gets to show the world AGAIN that he's the only one who knows what the fuck is what.
