Due to shit happening post-CCC endings, at least its believed currently, Nero became King of Mooncell and gained the Regalia that gives her absolute control of the Mooncell. If you pick Nero route in Extella anyway.
If you pick Tamamo its basically the same story, except its Tamamo who took over Mooncell post-ending. From what we know so far.
Whichever one you pick, the other one comes trying to fight your shit, and also declaring themselves king with their own faction of servants. So you have to take the faction of your servants under your command and fight them.
Some parts of this aren't sure at the moment, but some parts of it are known. Take it with a grain of salt for now. All servants at the head of a faction have a Regalia.

BB who devoured the mother goddess doesn't have the full power of the mother goddess, though, just the application of Potnia Theron, and only on imaginary spaces.

This time, they do it by summoning a bunch of dead Emperors who ally under Romulus and fund an opposing roman faction trying to conquer regular Rome. You join Nero in fighting them.
Consider that the pivotal point of the chapter you are asked to swallow is that "Romulus with guys like Julius Ceasar as aides would be a destructive force that would devastate history, and must be stopped" as opposed to letting Nero continue to rule. Did I mention the chapter opens with Roman mentioning how Nero will face an uprising and be executed some years after that time period? Because the writer straight up drops that on you and then goes on to write a whole chapter about you fighting to preserve her mandate as it being clearly the thing that leads history down the best path.
Then it's a bunch of pointless meandering until you meet Iskander, who is cooperating with the faction trying to devastate human history solely because he was curious and wanted to meet Nero. Also, Waver is willingly helping too, because Waver going fucking insane and abetting Iskander's stupidest fucking whims without opposition is kind of a repeating thing. Nero convinces Iskandar that she's fit to rule by beating him with your help, and then echoing his same brand of self-fellating endless-ambition tripe. I think the scene was meant to make you agree with them or something, in spite of the whole "This ends with you getting executed because it turns out that's not sensible state policy after all."
Then you meet Remulus, pull more or less the same shit and beat him too. Lacking any kind of fulfilling payoff, Attila the Hun is summoned as essentially a standard Final Boss with absolutely zero personality. you go beat her and then shit is over.
