If I had to guess I'd say because it was a different time, why would you as an artist draw guro now when you could get more work/money doing NTR for a h-manga publisher, also note that to make decent guro still requires a large amount of skill and detail it's far easier to draw people when the entrails are inside the body.

Because to draw decent guro you'd at least need a good idea of where organs located and what is inside the human body aside from just entrails and blood. Most doujin artists already fuck up basic human anatomy, so making eroguro is even harder. Breasts for example, aren't just blood bags that shoot blood if you cut them, but sacs of yellowish fat.
