This is one of those cultural things you're missing that the Japanese audience instantly gets.
It involves a concept called "giri" which can be roughly translated as "duty" and "obligation" and even "burden"
To put it simply, Ranma and Akane's engagement binds them by giri. To *not* go along with their parents arrangement would cause them to lose face in society.
You'll notice that they never deny being engaged, even early on in the series when they didn't *want* to be. Instead the most they protest with is "Our parents arranged it." saying to their peers that they're fiances because of giri.
Incidentally, this shows just how little hope Shampoo had of ever winning the Ranmabowl, since he publicly and openly refused to acknowledge her "engagement" to him. Ranma has no obligation to Shampoo by Japanese cultural standards, even if in her culture she does.
Of course, they are simply obligated to be *engaged,* We don't ever see Ranma and Akane have an omiai, so they could have one eventually. Or, if their first meeting "counted" even then, it's quite possible for the engaged couple to agree to break off the engagement without any real loss of face. Of course, this is very unlikely to happen, since Ranma and Akane *did* break it off once (with Nabiki agreeing to take on the obligation) and in the end decided to go back to being engaged.
