Have been watching the anime but this part has got me really confused.
Is this a stepladder because it has the parts you can step on? That is kind of strange though since you also have to step on a normal ladder to climb it. What is the difference between this stepladder and a normal ladder?

Not Ace Attorney's. They do exactly what localization was created for.
If Ace Attorney was handled by fucks from 4Kids this character wouldn't have existed in US's release or god knows would've been rewritten into an actual doctor. But they left him even though he literally doesn't serve any purpose except for pedophile jokes.
