Kaworu was created as his love interest. The episodes 22-24 were made simultaneously, 22-23 by some dude I forgot his name but were his only episodes, and 24 by the Eva scriptwriter Satsukawa (with Anno of course). Originally episodes 22-23 didn't kill off Rei and made Asuka comatose, so Shinji wasn't focused on his own angst but Misato's angst (about her loss of Kanji), so the idea was for him to experience the same kind of pain to understand her: fall in love and lose them. That's why Kaworu has some Kanji traits in body language. The romantic element was still present in the other versions: Shinji's palpitations and love declaration are intact.
Due to the huge popularity of the pairing (easily the trademark Eva couple in polls and such), Kaworu was changed from male to neutral (because he's Adam and Adam is 'female' of the species of angels) to make neckbeards more comfortable or something. Shinji still fall for a guy, he had no idea he was just apparently a guy but wait he's actually the Queen of Angels.
