Real spoilers, be ye warned:After they finally resolve her amnesia and discover the circumstances of her death, she's largely at peace with events and no longer held back by her repressed emotions. Being a ghost, that means she will pass on. There's a very touching scene in which the two of them are sitting in the clubroom saying their goodbyes, communicating through a notebook as each of them loses the ability to touch/hear/see the other. He resolves to eventually forget about her, move on, and be happy, after she repeatedly begged him not to make himself miserable pining after her once she's gone. He takes the notebook and locks it in a little box to keep himself from reading it and reminiscing.
Then in the last ten seconds or so she pops back up and says her love is so strong, IT'S now keeping her tied to the physical world. Roll credits.
