Eto mentions how ones creativity and intelligence shape their kagune, and I can only assume this applies to kakuja as well. Genetics could also play a role in it looking at Yoshimura and Etos kakuja being so similar, but I'm fairly certain he intentionally shaped his to look like hers.
Before, to Kaneki, being tortured by Jason was probably his most traumatizing experience, thus the centipede shaped his kakuja. But his wake up and the following BDSM session with Eto probably trumps that in some ways, at least emotionally.

Plus, as another anon pointed out, It'd be pretty silly to have his kakuja be snake like due to Nishiki. It also wouldn't make an outrageous amount of sense. The scales have been a visual cue Ishida has been using. It has no actual in universe connection to Kaneki, whereas the Owl has been hinted at pretty consistently.

The Kagune is always the main weapon of choice. Whether anyone has hand to hand abilities or not doesn't affect how strong they are. But I'm sure Ayato gave Hinami some pointers in that regard.

Most ghouls also shape their kagune into short range weapons of they don't do much hand to hand. Hinami is the only S rank to do neither.
There isn't much shut to say about Hinami she managed to hit an SS ghoul who had been jumping on her kagune then had what is claimed to be an amazing fight scene that wasn't seen in which she lost.
Ken must have been drinking that good good ghoul juice straight from Sphinx to get such a quality(tm) kagune.
Arima's done. He's pretty much delirious if he went at Ken using a lance as a sword. Possibly a limited lifespan from increased autistic abilities?

Not from what we've seen thus far. She hasn't had the chance to show off enough to prove she can or can't fight close range. Even if that was the case, she would use her Koukaku defense to help in that regard.

They're fuckin Koukaku. It's completely normal to develop hand to hand when you're Kagune relies entirely on close range combat. Hinami doesn't have that type, ergo not entirely necessary.

Because the battle lasted long. From the bruises and shit she had on her, it lasted long. Takizawa had to go Kakuja mode to deal damage to her and to heal up his severe wounds. And he was still going nuts because of it.
