I don't know if you've ever had a brother, or someone who you can love like a brother, but it is a relationship like no other. It's not a stretch. Kiku is either asexual or homosexual but he definitely not heterosexual. He could be in denial of his homosexuality. After all it would be considered very shameful to be gay and if he is attracted to Sukeroku he could be disturbed by it because Sukeroku views him as a brother and friend and he would be putting his relationship with him in jeopardy. I think Kiku would probably rather kill himself than be hated by Sukeroku.
Kiku seems aware of what is socially acceptable. For example when he was little boy he wanted to be a geisha, his older self scorned his younger self for having such pathetic goals. Also when he dressed up as a geisha as a grown man he was embarrassed because he knows is not acceptable even thought when he was a child it would have made him happy. Kiku is also aware of his surroundings and how people view him. That's why he bothers with trying to get Sukeroku to dress up appropriately in order for him to be socially accepted.
The most important reason is the author is a drama loving fujoshi who draws raw gay sex so there you go.
