Has there ever been a live-action adaptation from an anime/manga that was actually decent? I've tried a dozen so far and was left disappointed each time.

not really, the acting is really really bad, like what you expect out of japanese live action
The story on the other hand is pretty nice, they added some good twist to keep even fans interested and to shorten it to two movies.

The first 20th Century Boys movie was decent. The casting was phenomenal, almost uncanny in how they look like the manga characters. But then it just went down in quality with the sequels.
Chihayafuru looks like it should be good.
