The boy wants to kill the girl because she's older and more likely to die first of the disease, so her death would get more attention and the disease would be named after her, kind of like being a martyr for the sake of others who may be cured from what doctors can learn from the ones who have the disease, while the boy suspects his death would be a sidenote and may even have his negative qualities be the only thing that gets focus. Should the girl die from something else, she wouldn't get that attention. What he wants is to die from the disease and make other people acknowledge his sacrifice as the thing that saves their children should they catch the same disease.
We haven't gotten much info on why the girl wants to kill the boy, perhaps to ensure that she dies first from the disease or she'd kill him before he killed her, but that was only revealed at the end of the latest chapter.

4: He should know that he can't kill her without people eventually finding out that he murdered her. Therfore he will be remembered as murderer and not as "tragic hero" as he wants so it doesn't make sense to kill her. He could kill anyone if he wants to be remembered as murderer.
5:Why does he assume that her name will be left in history when she dies from the disease? Children have died before her and being charasmatic isn't enough. She might appear for one day in the newspaper.

1: Obscure in the sense that it was dubious. The doctor diagnosing the kid said he didn't even think it was real.
2: His disease and his wish were broadcast throughout the media.
3: That isn't a plothole, you're just nitpicking. He means individual as in unique, like a main character. If you've read Medaka Box or literally anything else by Nisio you'd be familiar with this angle.
4: He rationalizes feeling sympathy for the girl and not immediately murdering her by saying that would make it obvious he did it, so some part of him is trying to get away with it.
5: See 1. It's revealed in chapter 2 that he did jump to conclusions since, as you know, the girl's circumstances make her hard to publicize.
