There is a scene where one of her younger facial expression and one of her current facial expression got compared to underline the differences, which obviously makes sense if he remembered what she looked like.

The point was that it would have WORKED much better if Shaft would have given her very different haircuts and facial features during the flashbacks because in this way both Araragi's point of view and and the fact that he can't identify her every fucking time would have been more understandable.
