In my experience as a former fat person with fat family and have had fat friends, the biggest issue I've seen is ignorance. A lot of people are simply never taught anything real about nutrition and exercise and how it really works. I had to find out on my own about things like the BEE and the concept of keeping a food journal when I was about 16 or 17, and I just kind of chanced upon it. I had no clue up to that point that I could precisely calculate exactly how much I had to eat or not eat to lose weight at whatever rate I specified. In Clapistan, no one teaches you this in school, it's not in the media, even a lot of gyms won't teach you this shit. A lot of these people grow up hearing nothing but total bullshit about fad and crash diets and old wives' tales and never get any real advice from everyone in their lives and eventually they just get really jaded and give up. They're people that would keep up the most insane diets for a long, long time, many aren't lacking in willpower, they just lack in knowledge.

You're right that fatty foods are actually cheeper than most healthy foods (at least the kind that taste good), which is why poor people buy them, but a lot of the time it isn't that they don't cook out of laziness. For example, here in Clapistan there are a lot of poor people who work very long hours at one or more minimum wage jobs, and simply have no time to cook.
