To be perfectly clear, he's got a great deal of guilt becauseDuring the fire, he watched a lot of people struggle to save each other and die in the attempt, while he moved on to save himself. At the very end everyone dies except for him, and he's brought back from the brink by Kiritsugu just as he's about to accept his death
Long story short, it's not like he's crazy. It's just his way of staying sane is very unhealthy.

Guilt has nothing to do with it, that is only the surface and the explanation given in Fate route that it is completely proved wrong in UBW.

The reason that Archer is calling his ideal "fake" is because the only reason Shirou has that ideal in the first place is because he's just imitating Kiritsugu. While Kiritsugu had an earnest desire to save the world that came out of the tragedy on Alimango Island and all of the terrible shit he saw while traveling around with Natalia, Shirou's desire to save the world is purely the result of his old self burning away in the fire and the psyche of his new self copying Kiritsugu in order to survive and rebuild.

Nah Shirou contradicts that, and it's the major point in the story. His beliefs about the ideal was just as strong as Kerry's was. His belief in the ideal was not, it was not his born from him and as seen by the striking difference between the two he doesn't actually care if he achieving it. It was all about the path it was the path itself that captured Shirou so much whereas Kerry was obsessed with achieving the ideal.
Archer lamented that his path towards the ideal lead him to do what he didn't want to do in the first place, kill people, whereas Shirou accepts all this and continues because regardless of the end result it was the path he really wanted to follow
