The replayability is in being able to play the roguelike mode after you've mastered nightmare difficulty. Working your way toward that is probably the end-goal. Finding the secrets throughout the game adds a little replayability. Once you can mow down imps and possessed soldiers / hell razers without getting touched, shit gets real. Glory kills are pretty much necessary as a way to not waste ammo, and the pistol isn't totally worthless.
