To me it was the sounddesign. Even though in my opinion the game got worse as it went on and visually it wasn't all that scary past the first hour or so, the noises always had me at the edge of my seat.

The thing is Dead Space is objectively poor in the horror department.
All it ever does is gore and jumpscares, two of the cheapest and least effective forms of horror in existance. There is no suspense or psychology, nothing what makes great horrors great.
It's just a shooter with dark corridors and gory monsters playing dead or jumping out of vents.

I thought it was amazing at first but it starts dragging its feet very quickly.
It's heavily inspired by RE4, shame it doesn't have anywhere near that game's variety. RE4 feels like a thrill-ride and the game is constantly introducing new elements.
Dead Space is mostly "yeah here's a new enemy type I guess". Going zero-g was neat as fuck though, game needed more things like that to mix it up.
