It may not be the biggest but it's already more open world than Witcher. The whole overworld, from the start to finish with all the interiors, towns and dungeons, everything is seamless.
Not multi-regional like in Witcher 3.
That said, there is NO way FF XV tops W3's side quests but it looks like it does better in lots of other things.

I wish Elder Scrolls would step it up. I still like Elder Scrolls, but it's hard to really step down to that if Elder Scrolls 6 is still using Gamebryo and has awful melee combat. Not that TW3 had the greatest either.

I took a break after every expansion so that didn't happen to me. After this break, I'll continue my NG+
I tried it a bit after I beat B&W just to see. Glad I did. I avoided NG+ because while there is level scaling, all the quests are locked to level 30 and you won't get experience from monsters and such, so you're not gonna be leveling for a while.
Oh wait, that's what every fucking result in Google told me until I actually tried it and EVERYTHING scaled correctly. Maybe they updated or something. I'm glad NG+ adjusts correctly, though. Adds a lot of longevity.

Honestly, knowing SE, what concerns me about this open world is the attention to detail. Witcher 3 is overall excellent in this aspect. The NPCs have jobs, are hardly ever just sitting about, the terrain is fantastically detailed, with shit like a erosion and the pooling of water being placed somewhat realistically etc.
I don't see SE being that on point with their world. I think if you pay a lot of attention to your surroundings in this FFXV, you're gonna see a lot of static shit.

I know, it's my favorite. After playing through the game 2 times, I enjoy having everything look kinda dumb. I used to wear the long-beak purple mask, but Bunny Mask is ten times better. Mandrake picking gas mask is also good, but requires me to fuck over my bro Regis to get, so I never end up with it.

That's the way it is with all open world games that either become too big, or just don't have the same attention to detail. In the tw3, you can stop pretty much anywhere and find cool stuff in a 50m radius, whether it be locations, loot or just cool custom details like you mentioned.
Nothing makes me more disinterested in an open world if it just feels empty with no detail or interactivity and is clearly just big for the sake of being big.

Personally I just preferred doing most of the side quests in Velen/Novigrad since you're going to be way over leveled for most of them once you get back. There's a lot of cool quests and locations you've probably missed, like the whole Nilfgaardian army camp in the south of the map.
