Bioware just did a 3D scan of the lady and then put the head on a generic 3D body so the face didn't fit the body proportionally.
The problem is that the thing was just topography and poor resolution at that. Without people's natural skin coloring or any basic knowledge on how people look it looked like a misshapen potato. You see, 3D scans actually need some tweeking to actually look decent and that kind of effort is not what Bioware does.
