You lose all your current equipment, levels and everything in your immediate inventory which is certainly a penalty. They let you keep your stash and merchant levels so you can bring other characters back up faster, they also give you more loot in hardcore.
I'm against how they let you tune the difficulty to your ability and gear so it's entirely possible to never go below 80%hp by playing on the lowest difficulty, though you'll get shit loot and XP. Still, you shouldn't be able to eliminate all the tension at the cost of a little grinding.

Diablo 2 is a game I play every year. Its one of my favorites, but its engine is very old. 2 buttons for attacking, your buffs don't have timers or indicators besides the effect around your character, and some spells like charge and leap can still get stuck. It is what it is, and despite its aged interface, its still very fun to play. I recommend the Necromancer, I have yet to play a game that feels quite like a summoning Necro like D2.

Diablo II is the only game that even 15 years later I still haven't played all of the builds and there are still items I have never seen drop.
