The whole game dialogue is built like that.
This trash system forces four dialogue paths for every interaction possible. This leads to ridiculous shit like OP's pic where you're given four choices to actually say the same line, and situations where four paths aren't sufficient and a lot of possible dialogue is cut down because of that.
Add to that that this system is coupled with even worse writing than 3. 90% of dialogue options in this game sums up to Yes / More Info Yes / Sarcastic Yes / No (but actually a disguised yes). When a NPC talks to you, this system gives you the illusion that you're able to reply differently but the truth is that all four replies are the same with a change in tone and choice of words.
This system is one of worst things that ever happened to the series and Todd even admitted it was a mistake. This is why OP says "its mechanics are trash", the dialogue wheel is probably the best example of that.

Technically, including this and Automatron, you have paid for 4 story DLCs, so the fifty bucks you gave were totally worth it! Unleash your creativity!

I like fallout 3 and NV, ive never played 1 or 2 because im cancer, but they sound fun.
