The main premise of the game is the singleplayer. Co-Op is kinda hacked on with only the person initiating dialogue even getting it. I've never played the multiplayer but if I'm gonna be honest its probably not as good as NEOTOKYO or Dystopia.
E.Y.E is one of the best cyberpunk single player experiences, though.

Maybe thats one of the reasons? I'm sure designing your game around multiplayer instead of hacking it on later in development does make it more suited for multiplayer, I guess.
I'm not sure what this has to do with anything though. I simply explained that EYE's main draw is its singleplayer story and if you're looking for a multiplayer shooter in the same vein you're better off playing one of the source mods that accomplishes cyberpunk MP well.

There's no going back to the golden era that was the first week or two before they made friendly fire gone forever in co-op, just using the line of sight git fukd' psi powers on other people was incredibly amusing.
The multiplayer game modes they added on later are all kinds of fucked up and good for just being drunk as hell and not worrying about that dude who is moving at the speed of light with a katana trying to capture your flag while right behind him is a guy whipping around heavy armor and the minigun.

You have to do a bunch of port forwarding, which most dumbasses are incapable of, despite servers being up doesn't mean you can actually enter them.

Somewhat easy if you have access to your router to port forward. Though you have to disclose your IP address and need a computer good enough to host the server.
