Getting A+/S ranks definitely is. A lot of times it relies on enemies spawning with the right weapons and in the right locations.
Also people hate the windows in this game, but nothing has ruined a potentially good (combo) run for me than the FUCKING DOORS knocking a guy down and fucking up my combo chain. Also the game seems to prioritize picking up melee weapons over guns, which fucks me over a lot too.
