You obviously didn't. Here's a shot from a year ago with me getting level 35 round the time of the Kaer Morhen part. Note that this is before the battle and the massive xp gain you get from it, not to mention the endgame.
You could get to the point where the lvl 48 archgriffin in Skellige didn't have a skull icon in vanilla which I think is at level 41 or 42.

Enable level scaling if you want it though. Upgraded bombs and oils wreck and 2X distance whirlwind and rend are so effective that things live long enough to look stylish as hell but not too long it feels tanky

I am doubtful that he got that level and if he did it was through managing quests which he wouldn't have done on his first playthrough or without a full guide.

Yes I did. Because I already did 100% a long time ago, then played HoS, lost my save, B&W came in, I wanted to play it but didn't want that premade save so I started from scratch. All I wanted was to finish the main quest, get my manticore and go for B&W
