You can tweak a few things for a little extra challenge, add in new areas and full-fledged fan-made expansions, a complete overhaul of the entire game to make it more of a survival game and even add in all of Fallout 3 to play with the extras that New Vegas brings.

You can get stalked and raped if you walk around, there's a mod for that called Sex Assault, but the clothes you wear don't make any difference (apart from power armor wich basically makes you immune to rape)

I know of these mods and sexout framework, I'd like to know if anybody here actually played with recent versions of them and can recommend such a playthrough.

I only have stuff that doesn't mess too much with the game itself (nothing that modifies the story, quest or stuff like that) and it works pretty well
You have to fiddle around with the settings a lot if you run many mods

If you don't download lore-breaking stuff like most of the quest mods it's actually really immersive. I'm almost scared of walking around Freeside with my low level female character, turning around and noticing that someone's following you and is waiting for the right moment to tear your clothes off, inject you with a dose of med-x and have his way with you is really creepy

No, for the most parts. There are some okay animations, there are bad animations and then there are BAD animations.
