Everything including console game on my monitor. TV doesn't come close to a proper IPS monitor. Also since I'm on a capture card I can record and take screenshots with a single button. I can also run skype in the background together with the game sound.
Haven't used a TV in 6 years.

Depends on the tv, if it has a just scan feature I see no problem with using a tv. Personally I'd use a monitor though
I mean my monitor is like 8 years old, 23inch 1080p and allows component in etc. I used it for my 360 and now my computer. I think it's like 60hz. it gets pretty warm and loud and it's heavy/thick, but I love it.

Response Time its more complicated as some tv with high ms can produce ghosting and motion blur.
thats why l do all my gaming in a Plasma, perfect response time and great picture quality.

input lag, because it's not even a real thing. You mouse and keyboard don't magically send signals slower because of that tv/monitor you are using. It's should be called output delay.
2 monitors. One huge for gaming, second smaller for other stuff.
