The DOOM bible says that they were planning on a coop game with 4 different marines, each with a different personality. The only things that remained from the original pitch was the UAC corporation, an accident on mars and you saving the world from demons.

Doomguy is less a person and more a concept - The Grim Reaper of hell, so to say. They basically rolled with the decades of memes about him and his badassery and made it all true at the same time, to the point the game opens with a reference to the fucking Doom Comic. He is strongly hinted to be the same marine from the originals and possibly Doom 64, which through his exploits and battles against hell through the aeons became more than human and attained near-impossible martial prowess. His armor was also enhanced by a traitor demon at some point
The medieval Doomguys are originally from Argent D'Nur, one of the first worlds to be conquered and absorbed by Hell (Hell seems to have access to multiple dimensions at once). From their conquest, the demons attained control over Argent Energy, which the main antagonist in the game is siphoning from hell to our dimension to help with future humanity's energy crisis. Doomguy seems to have fought alongside the Night Sentinels at some point in his unrevealed cosmic entity past, as they appear as ghosts in the final missions lending you help
