Yeah it's a small bit of metal, but it's travelling so fast it does much worse than just stab wounds from a small knife.
The reason for this is because you have to get so close to the enemy.
So close that the chances of you being killed increase drastically. So the benefit has to outweigh the risk so melle damage is massive in most games.

I remember reading some report/article at some point where an experienced medic said that machete wounds were actually far harder to treat and had a lower survival chance than getting shot.

depending on where you get hit with something like a knife, cleaver, sword etc, getting fucking stabbled/slashed can be a far worse wound than a gunshot wound.
