Pureblood heartless are a side effect of people losing their hearts to darkness, seeping through from the dark realm. As this is a natural process, they are usually simple in form such as Neo-shadows.
Emblem heartless are manufactured through the use of dark power upon a heart, either through Ansem the wise, and later Xehanorts experiments and the MCP beneath hollow bastion/radiant garden. Or through the use of the dark power such as the villains in KH1 like Clayton and the Stealth Sneak or whatever it's called.
Nobodies are created when someone with a particularly strong heart becomes a heartless, with their spirit being changed into the Heartless and their body becoming the nobody. As they lack a heart, they are without emotion. (Roxas is an exception to this as he possesses Ventus' heart.)
The nobodies who form Organisation XII are the nobodies of exceptionally strong hearts, this may be developed later in KH3 and have something to do with the seven heros of light and the ones of darkness respectively.
