Just because something has obscure or hard to understand visuals or motifs doesn't make it pretentious. Hell, look at any surrealist painting and there's a shitload of stuff going on you could construe as pretentious despite their actual meaning or nuanced references/influences.
The game's visuals clearly reinforce themes of death and rebirth. The desolate beach, the massive amounts of dead sealife washed ashore, Reedus' shackles, etc. It's clearly some purgatory or afterlife. He obviously birthed that kid via c-section, making it an "unnatural" birth, and the invisible inky baby hands further reinforce the unnatural state of things. Then the watchers in the sky. It's obscure and hard to figure out but not everything needs a perfectly accurate meaning or reference, sometimes a visual is just a visual intended to affect a mood or tone. It's not pretentious.
I fucking love Kojima as much as the next person, but this fucking trailer proves how far up his own ass he is. He literally believes he's God's gift to mankind. He thinks he's some kind of auteur like Alejandro Jodorowsky. I think all the praise he's gotten went to his head.

Every artist has things to say by virtue of being an artist, their work is how they express themselves. You have to have a degree of self importance when your actual career and livelihood is built upon your self expression. You have to be valid in the assertion what you have to say to others is worth their time.
Some artists have simple things to say (see Monet and impressionism) but some have very complex, nuanced things in their work (see pic related, it's a portrait of the personality of his best friend/possible lover, a German cavalry officer killed in the opening weeks of WWI. He opted to not paint his face or likeness but things that constituted his personality, dreams, desires, etc. in the wake of his death). You could call this very pretentious if you don't "get" it or know what's going on but when you pull apart everything you see it all has a meaning and the artist had a whole lot to say about his friend - that his image wasn't him, but THIS is who he was. His age, a checkerboard for his favorite game and pastime, his rank and insignia, his flag, initials, things he was proud of and aspired to be, and what killed him.
Just because you don't understand a work of art at first glance doesn't make it pretentious. Sometimes it's good and healthy to not fully know what's going on, to wonder and think it over, come to your own conclusions. Art is as much experiential as it is an expression of the artist. It's why, despite not having really much to necessarily say, Monet's works are still beautiful and inspiring today.

Understand that the hate for Konami doesn't soley come from kicking out one auteur Director for an over-budget and behind schedule game.
