You want to see what the game will look like? You do know that games are made differently than movies right? Kojima makes a trailer for people to guess and WONDER (oooh wow) and speculate. He will then LATER show some gameplay and then you can decide whether to steal mommy's money or not to buy the game.

First streams of gameplay prolly won't come at least until summer of next year. But with Kojima having a new/(maybe)larger team and a new company to provide cash, it could be faster. Why does this trailer bother you so much? They didn't even give a release date.

The problem is that (on top of having narrow eyes to begin with) he got in a really bad car accident like 10 years ago, his right eye socket got shattered and replaced with titanium that makes it look puffy. You can even see in OP image, it's usually apparent if he's sweaty or in harsh lighting.
