Okay, I have to level with you /v/. I am bad at games. All games. Regardless of genre or skill floor or skill cap, I am bad. And it's depressing as all hell. How do I get better? I've but 100's of hours into some games and still find myself below average at best. Am I doomed for pathetic failure my whole life?

Hard to know without any more info, like if there is some physical root cause. I badly injured some tendons in an industrial work accident a decade ago for example, so there are certain games (like fighters) that I'll probably never be able to be that good at, though I can still have fun. But I worked with doctors to figure out some extra treatments, exercises and such and researched changes I could make to controls and learned to help compensate for it a bit. When was the last time you had a full physical? If you've got some underlying body problem then you might be able to improve there, although some stuff might take years.
If you are completely fine and healthy and still suck, all you can do is keep practicing, perhaps though focusing more on much smaller things to get better at. Like, pick one genre, something that doesn't require massive reaction time maybe like TBS or RTS, and watch videos/read a lot and try to get really good at one piece. Learn how to beat an AI or whatever, then harder and harder levels, then faster and faster. Focus on a few strats at a time. I'm being vague here because I don't know what you want to do, but break it down basically, maybe you're getting overwhelmed by trying to do too much and spinning your wheels putting a lot of time in without really improving. Certain genres have competitive communities with good wikis and FAQs and boards with advice for newbies looking to git gud, try some of that. Focus.

Also maybe try going sidewise into something completely different for a bit to try honing yourself, then return to normal vidya. Like, take up Go/Chess/Mahjong or whatever for a year or two, do memory exercises and the like, things that 'll just help you generally. Maybe that'd help with your rut.

I don't think there's a physical cause. I have no health issues along those lines that I know of. I just have really poor reaction times and I feel as though I'm slowing down mentally, probably from depression or some shit.

Eh, that can be physical too. I mean, if you're just a bit "down in the dumps" then whatever, but if you've got some neurochemical cascade failure starting up that'd be worth getting fixed. If it lasts for weeks and is affecting your actual life in general then get it dealt with there, there's nothing cool or mature about not dealing with sickness.
If you're just down because of suckage though then I think a focused approach on small improvements may be a good place to start, or as I said switching frames for a bit. Depending on your current exercise regime you might be able to make some helpful changes, a lot of pros have specific hand/wrist stuff they do. Google for stuff to practice reflexes.
Whatever you decide make sure it's something you can MEASURE, because in the real world improvement tends to be very slow so it's really valuable to have a computer that can show you you are objectively getting better, even if it's below perception. When I took up rowing for example I started with a horrible time, but improved watt by watt over years. Now I feel pretty good, but I don't think I would have maintained it without a computer tracker. Consistency is pretty key.

one of the few exceptions to this suckage is grand strategy like Civ, where it's turn based and I can truly think about what I'm doing before I do it.

I've tried the "small improvement" method, but often I don't even know where to begin with most games. Usually on having a good understand of mechanics but that never helps me get better, in fact it usually makes me feel worse.

Pick a game you really really want to be good at. Keep playing it every day until you've beaten it, using whatever means possible (easy mode, co-op help, following a guide etc.)
Keep playing the same game again, this time handicapping yourself a little more each time, until you've mastered the mechanics.
