There was something similar in FEAR 2 early on where you're supposed to jump into a pool. I was going to look for another path but I found Alma watching me silently when I turned around. There was no sudden loud noise or some music note going off, just her standing there, watching. I ended up backing away from her into the pool where the typical scripted scare happened. I've even looked up playthroughs to see if anyone else encountered it. Everyone just immediately jumps into the pool.

I can distinguish just a bunch of characters from the dialogue.

There is actually a really funny glitch in that area where you can look at Alma and she won't vanish.
Jump into the pool and trigger the cutscene, once you're out of the pool go into the next room and then jump back down to the pool and make your way back to the vent you just came out of.
Alma will be standing there and she will follow you around with a creepy animation. She also teleports to you when you look away.

She vanishes once you hit a checkpoint if I remember correctly. But there are spots in FEAR 2 where she's behind you and the game gives you no sign that she's there. It's neat.
