Roadhog is great because he's actually everything that Reaper tries really, really hard to be. He's a guy who terrifies people, places little to no value on human life, and could not possibly care less about what society thinks of him.
Reaper really wants people to think he's a cool, uncaring badass so he carefully crafts this image of himself with the mask and the dark coat and the engraved shotguns, and a list of edgy things for him to say that he thinks are cool. And everyone thinks he's a fucking joke and laughs at him all the time.
Roadhog doesn't like shirts so he doesn't wear one. He does like pigs so he tattooed a pig head on his stomach and wears a mask that looks like a pig. He doesn't talk apart from muttering weird shit about the apocalypse. He huffs weird shit out of gas cans and when he's bored he murders people with a hook and shotgun that looks like he built it with shit he found in a dumpster. And the only person who ever dares to even talk to him is Junkrat, who is literally paying Roadhog not to murder him. Everyone else just stays the fuck away from him.

I would actually love some proper autistic characters but you dont really see them. The problem with the whole autism thing is that its used to pander to people who dont have it (and SJW who think they do). Borderlands 2 had an autistic character and she was actually decent considering. The problem with most mental illness in games is it just feels tacked on or like a shitty trope. If it can add some level of depth to the character it actually works really well.
