A lot of his interactions in WotLK were handled poorly because everyone treated him like he was still Arthas Menethil (they called him Arthas all the time too), which was stupid. They should have realized he's some sort of undead abomination and no longer a human being that can be treated as such. It did this so often it actually succeeded in making him seem less threatening, since evil human guy is a lot less scary than Lord of the Damned.
But I'd still say he's ranked pretty high.

That's because all the people interacting with him in WotLK mostly/only knew him before he put on the Helm of Domination, i.e.: when he was still fully Arthas Menethil.
