Within video games? Nah. Fallout New Vegas' survivor story from Honest Hearts easily beats it and that's just off the top of my head, there are way more.

Within Persona? Nah. Sun social link with the dying writer is much sadder. The ending of that link especially got me.

Death in P4 is alright, but honestly a little frustrating in how much metaphor is thrown around to make her seem mysterious. Perhaps it's just poorly translated and the dialogue has more impact within a Japanese cultural mind set, but regardless a lot of the dialogue fell short.
