Haven't played W3 since HoS and jumping straight into Toussaint didn't feel that differen't, just a bit more blue. Then I went back to fuck around Velen and Skellige and everything from plants to atmosphere is completely different. Alchemy tree is incredibly strong, but to really make it work you need decoctions (technically only 2 if you want to abuse Archgriffin and Ehkidna), potions, oils, bombs, armor, etc. It's a build that works well late game, I switched to an alchemy/partial sword build when I started Heart of Stone and I never looked back, Geralt is damn near invincible with the build I'm using.

I started NG+ right after Hearts of Stone, and a month and a half ago I went through all the main story + HoS to be ready for Blood and Wine. I finished HoS just the day before release, and I had an overall hard time but pretty enjoyable on Death March. A few points are top tier bullshit in Death March + NG+ tho (Olgierd, Iris Greatest Fear, BaW final boss phase two special attack, boxing matches).
Those were the weak points, but they forced me to do a heavily specialized and focused build, and aiming for OP Geralt to have a chance to play nicely. I was forced to use oils and more potions and bombs than on NG and regular difficulties, so there's that. But you never end being too much overpowered, mind, even with Runewright, fully greatmaster witcher armor and weapons. Aerondight is a teeny bit wee overpowered though, I'll give you that.

I would say it's a hit or miss with the side quests and most of them seem to be a miss. I have encountered a few that were good and up but there were only a handful. It was just the planet titling on its axis and making the planet become inhabitable in 3000 years. The prophecy was there to flesh out the elder blood and to give reason to why some people wanted Ciri.

An interesting thing about Eredin from the books was that he just wanted to help the Aen Seidhe escape from the Witcher world before the white frost destroyed. Hell, he even said if he had to he would bring along the humans and the other races. Also he didn't mean to kill the king.
