You don't stack HP and lifesteal. That's horribly inefficient.
Damage reduction and HP is the most efficient combo. It synergizes the best with healing too, except maybe a damage reduction+dodge combo.
HP and dodge/avoidance is the riskier option. It can either be better or worse depending on RNG. Healers will hate your guts because of how much you HP bar will swing around.
Damage reduction and lifesteal is a more aggressive combo. It's not as effective, but it's tricky since you give the impression of being closer to death than you really are. Doesn't really benefit at all from a healer's help.
Lifesteal and dodge is "I want to feed" tier.
HP and lifesteal is "I want to feed AND be useless" tier.
Damage reduction and dodge is "I don't care about winning, I just want to be an asshole and make everyone hate me" or "I have a pocket healer, get fucked losers" tier.

If plate gets even one dodge(which is more likely since it can take more hits) then it already beats leather.
Plate is better for someone who will be taking a lot of attacks. Leather is better for someone who only gets attacked once or twice here and there.
