When Venom Snake rescues him, even if he is totally blind (which he isn't), Ocelot mentions his wounds to Miller.
Miller knows from the beginning. The final conversation takes place before Ocelot hypnotizes himself, probably right after Miller gets rescued. He spells out his motivation pretty clearly: he wants to make VS stronger in order to have him take on Big Boss.
After the game he probably realized there was no way VS would betray Big Boss and left in order to train Solid Snake. If they ever made Chapter 3 Peace (Kazuhira) it probably would have been about him and VS eventually growing distant and him leaving, I guess.

It looks like the exact same eyes the parasite units have. It's even mentioned that Miller was captured by them and it's played for dramatic effect when he opens his eyes. Not to mention Venom comments on his eyes and Codetalker tells him "eyes on Kazuhira."
I thought it was going to be revealed he was infected with parasites, but no. it's never touched on again. Kaz's eyes seemed like an important plot point that never got cut.
