You can punch or shoot the bamboo in that area to get leaves to fall off of the plant. Same room as the ice cube bucket.
Hold down the select item button, highlight the ration, and press circle (punch). Otherwise, Snake will automatically use one up when your health is depleted
When you're hanging on a railing, you can press R1 + L1 to pull yourself up. Do these pull ups 100 times to increase your grip level. Do another 100 to increase again.

Aside from reflex, yes. In terms of your options and the amount of creativity you can employ to distract and neutralize guards, the series only improved with each game. Peace Walker notwithstanding, but it's a portable game and designed around those limitations.
