The first panel, where they are both younger, show their relationship in the original Overwatch days: Ziegler the medic who insists on performing triage to their leader Jack Morrison. Jack acts tough and says his arm wound is nothing special, go treat the others. Ziegler has none of it because she's the medical officer and in medical matters, the doctor outranks always.
She also loves him and doesn't like seeing him suffer.
In the second panel, many years have passed and Ziegler, now known as Mercy is still treating Jack, now known as Soldier 76, for the same wounds because he's still getting hurt. The biggest difference is, since it's the future, you see that Jack has been fighting very, very hard on the frontlines the whole time and bears innumerable scars and heavy wounds. She can only heal wounds, but can't treat the scars and it breaks her heart the man she loves is constantly hurting himself and fighting and she can't force him to stop. It's killing him, and while she's in the business of saving lives, she can't save the most important person to her.
The point is, for as good a healer Mercy is, she can't save the most important person to her as he is slowly dying and she understands she'll outlive him, becoming alone.
I teared up in MGS2 when Jack told Rose about how he was a child soldier and Rose cried for him.
