The Actual-Code explanation for why missingno happens is so fascinating.
When the game does the Viridian Trainer demo, it has to move his name into the display memory, so it moves yours temporarily to another memory address so it can be retrieved later. Since you're in a town, the "wild pokemon RNG registry" address is vacant, so the game uses that.
Then if you fly to Cinnabar, you can surf on that stretch of land while still being IN Cinnabar, without ever entering a Route that has a wild pokemon registry (this is why that same stretch of land can be used for Safari Zone pokemon capture outside Safari Zone) so the game never writes anything new to the wild pokemon registry memory address.
Since your name is in the address, the game does its best to gather pokemon data using that as the table, but it's invalid, so instead it starts reading memory addresses it shouldn't be reading trying to find the lookup sprite data for the mon and lookup audio data for the cry; that's why the game draws a random assortment of tiles, and the invalid cry overflow (IIRC) is what causes the 6th item slot to duplicate.
That's also why people with certain names can see certain sprites like the fossils or ghost for Missingno, and also why in Blue and Red it tended to have different names ("MissingNo." and "M'" being the data called up when common names like Red/Ash were used vs Blue/Gary were used)

Simple: it wasn't as common as you think it was. Since it had some basic reason for existing, namely the truck being there for no reason, people theorized about it and with the special nature of Mew meant that it was the focal point of many theories. It sprung up on its own in multiple instances.
However, it wasn't universal. There were multiple theories floating around my school as to the existence of Mew, but the truck wasn't one of them until around Middle School, when people actually did have the internet. The seemingly universal part of it is that once you heard about it later on, it sort of replaced memories of what actually happened. For every time you heard about "mew truck", remember when someone told you that you had to trade surfing pikachu with some trainer that didn't exist, and any other theory that made no sense? Mew Truck was just the most resistant to testing.
