7970 still rock solid 4 years later. Upgrading probably next gen with some newegg bux IF I see games I want that need it that my 7970 can't reach 60 FPS anymore. Fallout 4 is horribly optimized anyway. If that's the main game you want to play, yeah you need to upgrade. But F4 is also really CPU dependent, and I wouldn't reccommend changing out your 8350.
Force tessellation to 16x in AMD control center, turn off gameworks BS like God Rays. Turn down settings at least one notch, becuase there's almost no difference between high and ultra. Look at the 1080p GPU and CPU graphs here

Yes, that's an old cpu (I enjoyed mine, though). The 8350 is also good, but I'd recommend going for a beefier haswell at least, it'll last you longer. There's no decent upgrade from the 8350, and you'll have to buy a new mobo when you do (probably true no matter what cpu you get, but if you get an equivalent haswell cpu you can still upgrade to a significantly better haswell without upgrading the mobo too).
