That's true, but they're also slow. At long enough ranges to reduce damage from hitscan you're not going to get consistent directs if they know you're there, and they're going to chip you down. It's always a favorable engagement for Soldier 76 since even if you both cripple each other he can heal, and if McCree hits you in the head even once you have to drop immediately or you're dead.
Also, don't forget that 76 has a rocket launcher, too. Helix Rockets are way the fuck faster than Pharah's and do the exact same damage. One direct and you're fucked - and since she's so slow in the air it's actually easier for him to get a direct on her than vice versa.

At those ranges, it's a matter of experience with the rocket launcher. You have splash and only really have to track them on one axis, they have to track you on two. You probably shouldn't expect a kill at those ranges, but you can easily put heavy pressure on them (and anyone else near them) and force a disengage, which allows your team to push forward. Pharah's mobility and unparallelled Z-axis gives her exceptional map visibility and lack of range falloff should let her pick when and where to harass defenders. She's not as strong of a pick on Defense, but still viable if the enemy comp is particularly heavy on short range characters like rheinhardt, mei, tracer, ect. Pharah can produce enormous damage at range, but lacking any sort of headshot or secondary weapon, her burst means her TTK is very disadvantageous at close range unless you can absolutely nail the juggle and control the engagement from the start.

Lots and lots of practice. If Rocket Arena 3 is any indication, direct hits from the ground are a losing game before 250hr or so of practice, and even then it's going to be reliant on luck and enemy stupidity.
Thankfully, Pharah can easily come at opponents from above, which means even if you're close, you'll hit the ground right next to them, knocking them to a predictable location. This is how you kill people as Pharah. Your first rocket should be aimed for where they will be, your second rocket aimed for where they will be after being knocked back from the first, and depending on if your first actually connected or not, your third will be a kill shot or your first retry.
Concussion is like a cheatermode first rocket if you're using it offensivley. It's MUCH faster, the knockback radius is double a normal rocket, and the knockback is stronger. You can easily use this to position someone for a rocket juggle. Knocking them into a corner is basically cheating because you just have to lob rockets at the corner and the successive knockbacks will force them back into the corner until they die.
