To be comparable to the Max Headroom incident, it would have to be an event where the game experience was profoundly tampered with by a third party outside of the control of the publisher. This brings to mind various cases where people gained access to admin privileges in MMOs, or perhaps the flying dongs incident in Second Life (though you could argue in that case that everything was "working as intended" and Second Life isn't VIDEO GAMES anyway).
Can't really think of a case where this was done for non-multiplayer video games on a large scale. Perhaps you could gain access to some indie developer's web hosting and replace the binaries for their game with a modified version. Short of that, I guess you could go around to Redboxes replacing the disc in every copy of a certain game with some other disc as a prank.

EAS always freaked me out because not only is the alert sound so abnormal but speech is always distorted.
