It sets random foot patrols between outposts and basically all over the world, it adds a couple of helis flying up high and sometimes coming down to check outposts and bases, it makes it so there's more vehicles driving around, and so you have tanks, APCs and such instead of just trucks...
You can also set yourself to go in Subsistence mode, adjust parameters like your health, the equipment of soldiers in the field, make it so it's a game over if you ever get detected even on free roam, etc.
