Twisted Metal Black takes place in Sweet Tooth's head, it's all a dream/nightmare. This is revealed in the last level after you play through the entire campaign as minion, his loading screen gives information in code to the player and he warns you that it is not real.

This gets even stranger when you realize Head-On is essentially TM3. [Sweet Tooth is really Marcus Kane. As we all know in TM2 the whole series is a dream. Even in TM Reboot Sweet Tooth's real identity is shown. Indicating that Sweet Tooth is Marcus' repressed emotions and Twisted Metal is his fantasy. In reality he's probably a homeless man with mental illness who lives in a car. Oh and Sweet Tooth never sleeps in Black yet Marcus awakes in 2. And in Head On they become one which is already basic fact by Reboot.] Twisted Metal is shockingly deep.

The secrets in this game were very well fleshed out for a kids game, I was genuinely surprised going to the other older twisted metals to find that it was something that they lacked.I have probably around 600 hours into this game spread throughout the course of my life, I just keep coming back to it, 550 of those hours was when I was younger and playing it just about every day for 2 years and the last handful are coming from recent replays to bring back the nostalgia
