I got myself the "Insane in the Membrane" title in WoW.
But that ain't shit compared to the autism I poured intomodern RuneScape. They released a new mode, Ironman, a few years back. Basically the same game but with no trading possible (even via pking), no GE, no pay to win, everything is entirely self sufficient.
I started a month after the modes release (I didn't want to play it originally), but even with that huge handicap I managed to reach page 1 on the high scores (top 25).
The grindy skills are infinitely more grindy when you've got such limited ways of leveling them.
And to top things off, I did it in the "Hardcore" mode they offered - same thing but you pretty much only have one life (the several quests that take place in the wildy were pretty intense).
I don't know why the fuck I was so into RS3, thank God I got out of that time trap.

Trying to get Feebas in Ruby and Sapphire was soulcrushing.

It's not that hard, just make sure you get the Spur and never EVER grab the missile launcher. You want health, not damage.
I did close to all of the dumb grinding shit in WoW that was available to my character at the time I began playing. Every mount I could grind for in Wrath, every faction, and the original Insane in the Membrane achievement. I was also unsure about the rules regarding Steamwheedle Cartel and Bloodsail, so I did the grinds assuming I had to have them at Exalted and Honored respectively at the same time, rather than just once at any time, as I believe the case was eventually confirmed to be.
Don't play the game anymore. But it's a sunk cost, no point regretting it now.

In WoW, I leveled a character on a dead server high enough to get Fishing as high as possible to do the Booty Bay Fishing Contest because my main server was overpopulated so would always get shit on every sunday.
Deleted the character afterwards.
Salty title was worth it.
Mined gold ore in RuneScape for 4 months straight to get 99 Mining & a shit load of money.
Fished sharks in Catherby for 3 months to get 99 Fishing and a SHIT load of dosh.

Anyone else find the Witcher 2 boss battles later in the game to be really hard on normal? Even certain enemies but I don't know if it's because I'm using a regular sword or not. I recently played it where I left off of at the krayan and literally two strikes abd poison and I'm dead. Problem is when you switch to easy the enemy never fucking fights you and it's easy as hell. I always play games on normal/medium. It might be that it's been a couple months since I played it and maybe forgot a few things.

I spent about a week farming for Equinox in Warframe. You need to collect 8 blueprint pieces compared to the normal 3 for other frames. Even then, once you have all the pieces you have to wait just shy of 7 days for it to actually finish building. Light compared to other grinds, but I still lost track of how many times I killed Tyl Regor.

In WoW there's this thing called the Proving Grounds, basically you have to beat waves of NPC's as a certain role (tank, healer or damage) before you're able to queue for heroic dungeons.
It's so some clueless idiot can't queue up for that sort of stuff and screw the group over.
What I did was complete the silver proving ground (the second hardest - the one required for heroics) for tanks, as a rogue.
That might sound simple, but my God it was tough. Every single move had to be perfect. It's hella hard trying to gain aggro on groups of mobs and mobs that can insta kill you. There's little in the way of randomness there, everything just has to be perfectly executed for those 7~ minutes.
I must be one of a very, very slim few who managed to do it (I'd imagine around 5-10 worldwide).
There's no title, no mount, no real reward. I'm the only person who will ever know.
