Yes you can either start the expansions straight from the menu and they give you a appropriately leveled Geralt, or you can go to the expansion with the Geralt you've played through the base game.

I only played witcher 3 up to the point where the armada has gathered at skellige, then I got exhausted and was overleveled, and quit.

It's only really hard for the first 30 hours or so, then the difficulty curve becomes virtually non-existent because of some poor balancing. If you find it's a bs boss battle just lower the difficulty and progress further if you still have interest.
