Even after the patch and rebuilding database this game is still having graphics issues. Don't know what to do, I just wanted to enjoy my game

Your PS4's GPU looks like it's dying. It's a common issue with AMD GPUs, they have a very high temperature operation so the physical components of the GPUs themselves actually start to deteriorate and finally die.
Time to get a new console.
