It's derived from the earliest of tabletop games where you were assigned a role and played that role, but along with it came things like dice rolls, turns, classes, races, and statistics. Those concepts were the foundation for the video game form of RPG. The "role" aspect is merely the basis rather than the focus. It's not an RPG if it lacks combinations of the other elements I mentioned.

The entire reason an RPG is different from typical Roleplay is because of statistical management that turns your Role Playing into a Game. The reason why DnD is different from Larping is because there are game mechanics to determine exactly why your characters do what they do and what they do within it. Your stats determine your character's ability to succeed within a scenario moreso than just being a slight modifier from other events.
A character in your average Wizardry or Wizardry-clone game will only succeed in battle if they are not only strong enough, but are also making use of statistically superior abilities and gear along the way. Link in LoZ will only succeed if your personal ability to control his abilities are sufficient to complete the task. That's why the former is an RPG and the latter is not.
You can argue the degree within something goes between the RPG-"Other Genre" gradient(Dark Souls can be completed with overwhelming statistical superiority as well as superior technical skill and knowhow, with both being an ideal pair, so it is an even Action-RPG), but the number of dialogue choices is wholly irrelevant beyond the Roleplaying element of it. The actual Game is why statistical management is inherently necessary and integral to the definition of the genre. A Visual Novel is an example of a game devoid of RPG mechanics and only filled with Roleplaying, similar to a Choose Your Own Adventure book, but with superior visual effects and audio. "Decision Making" has nothing to do with what makes an RPG an RP"G".

City of Heroes felt 10 years old a year after its launch, looked like it was 6 years from the future and was made alive by the most thriving community I've ever seen in a game. Its the only MMO i've played roleplayers never broke character and the majority of players were roleplayers.

The thing I find in these games that I don't see in every RPG is pure chaos, that feeling that anything can happen.
Backer content was kind of shitty but it gave me that city of heroes feeling, like paging through idiots character profiles to look for the cool ones.

I love Kingdom Hearts but I don't think it qualifies as an RPG. You have next to not control over how your character evolves. In some of the games you have a little more control (BBS, Days, CoM) but it's really not all that much. And the 2nd thing is that the story is the same for every playthrough.
